If you have been dismissed from Luther College or withdrew from the college while you were on Academic Probation, you will need to complete both the Dismissal Appeal and Readmission form and the Academic Success Plan (ASP).

If you are a continuing student on Academic Probation, you will need to complete only the Academic Success Plan (ASP).
