- Student Health Evaluation Form
Luther College requires all students complete this student health evaluation form as part of their admission process. This information will help us support student health needs during their time at Luther College. Download the form to complete and submit using Norse Hub. Complete instructions are included on the form. Student athletes will be directed to complete additional forms distributed by the Athletic Training department; including an athletic physical. Athletic forms are to be submitted to Luther's athlete portal (not Norse Hub). Questions should be directed to the Office of Student Engagement. - Immunization Waiver Form
This form is to be printed off and filled out by all students requesting immunization exemption. Please fill out the entire form as directed and return to the Office of Student Engagement. - Religious Immunization Waiver Form
This form needs to be printed and completed by student requesting an immunization exemption for religious reasons. The completed form should be returned to the Office of Student Engagement.
