Please use this link to securely submit your W-4's. Do not email them. Your I-9 must be completed in person in our office and your Direct Deposit must be delivered to us in person. 

Required

• Federal I9 Form – This form is available on campus and must be completed before a student can begin work on campus. Please see this list of acceptable ORIGINAL documents that must be presented to the Student Employment Office when completing this form.)
• Federal W4 Form – necessary to calculate withholding for federal taxes.
• Iowa W4 Form – necessary to calculate withholding for state taxes.

Illinois Residences Only

• Illinois W4 Form – necessary to calculate tax withholding for Illinois residents only.
• Non-residence Iowa Tax Form (PDF) – necessary for Illinois residents for tax reciprocity.

Optional

• Work Study Payment Agreement – required if the student plans to have earnings applied towards institutional charges.
• Direct Deposit of Wages – required if participating in electronic deposit of wages into a student's bank account.
